Physiotherapy notes software

    Physiotherapy notes, written in minutes

Talk through the assessment after the patient leaves and NoteSono writes it up as a physio note for you to check and sign, with the body chart, red flag screening and home exercises kept alongside. For UK physiotherapists, sports therapists, osteopaths and chiropractors in private practice.

    Free plan, no card needed

    In short

NoteSono is clinical notes software for UK physiotherapy practice. After a session you dictate or type what happened, and NoteSono drafts a physio initial assessment or follow-up note, from presenting complaint and objective examination to treatment given, home exercises and plan, for you to check and sign.

Mark symptoms on a front and back body chart, record the red and yellow flag screening you did, and give the patient a home exercise sheet as a PDF or in their portal. Records are held in the EU, and there is a free plan with 5 notes a month.

    • Physio assessment and follow-up notes
    • Body chart and flag screening record
    • Home exercise sheets
    • You check and sign every note
    • Held in the EU, ICO registered

    How it works

From session to signed note in a few minutes

Three steps after each session, and the admin around it is done.

    1

    Talk it through

After the session, speak for a minute or two: what the patient told you, what you found on examination, what you did and what they are doing at home. Or type it, or write it section by section with no AI.

    2

    Check and sign

NoteSono drafts a physio assessment or follow-up note, keeping every measurement, side and grade as you said it. You read it, change anything and sign it. A signed note is locked; later changes go in a dated addendum.

    3

    Chart, flags and exercises

Mark the body chart, record your red and yellow flag screening and update the home exercises. All of it is saved with the patient record and printed with the note.

    Built for physio notes

The note a physio actually writes

Talking-therapy formats do not fit a musculoskeletal assessment. NoteSono has physio formats with the sections you use, and keeps the numbers exactly as you said them.

    Physio initial assessment

Presenting complaint, history, symptoms and pain, red and yellow flags, objective examination, clinical impression, treatment given, home exercises and plan.

    Physio follow-up

Subjective update, objective reassessment, treatment given, changes to the home exercises and plan. Discharge summaries use the discharge format.

    Your findings, not ours

Range of movement, strength grades, special tests and sides are written as you stated them. The clinical impression is only ever yours: NoteSono never offers a diagnosis.

    Body chart

Mark pain, ache, burning, pins and needles, numbness or stiffness on a front and back chart, with a detail for each mark. Saved with the note and fixed once you sign.

    Red and yellow flags

Record the screening you did, item by item, as yes, no or not asked, with your comments. A record of what you asked: NoteSono does not assess or suggest anything from it.

    Home exercises

Sets, reps, hold, how often and instructions for each exercise. Download a PDF to hand over, or the patient sees their programme in the client portal.

    Who it is for

    Set up for physical therapies

Choose your profession and NoteSono sets the words, session types, note formats and measures to match. You can change any of them.

    • Physiotherapists (HCPC, CSP)
    • Sports therapists (BASRaT, SST)
    • Osteopaths (GOsC, iO)
    • Chiropractors (GCC, BCA, RCC)
    • Patients, not clients, throughout
    • Initial assessment, follow-up and discharge review session types
    • Pain scores and activity ratings in the session

    Running the clinic

Notes next to the diary and the invoices

On Pro, the same system runs your diary, online booking, reminders, invoices and card payments, and insurer work: record the authorisation, track the sessions used and send invoices to the insurer.

Also on Pro, GP letters, referral letters and insurer reports are drafted from your signed notes for you to edit before they go.

    Choosing software

Questions to ask any physio notes software

Where is the data held? Health records are special category data under UK GDPR. You should be able to name the country and every sub-processor. NoteSono holds records in the EU.

Does the AI train on your notes? It should not. The AI providers NoteSono uses do not train on your content.

Are signed notes locked? A record you may need to defend should not be quietly editable. In NoteSono, corrections go in a dated addendum.

Can you leave with everything? NoteSono exports every note, patient and invoice on every plan, including the free one.

    Privacy and safety

    Clinical records handled with care

Built for UK practice, with your professional body's expectations in mind.

    You stay the clinician

NoteSono writes up what you said, in your format. It never assesses risk, suggests a diagnosis or recommends treatment. You check and sign every note.

    Held in the EU

Registered with the ICO (ZC045529). Records are held in the EU and encrypted. AI providers we use do not train on your content.

    AI only with consent

Record each client's consent to AI. A client who says no can still have notes, written by you without AI, and you can switch AI off entirely.

    Your records are yours

Export every note, client and invoice at any time, on every plan, including free. If you stop paying, nothing is held back.

    Common questions

Does NoteSono have physiotherapy note templates?

Yes. Physio initial assessment and physio follow-up formats are built in, with sections for presenting complaint, history, symptoms and pain, red and yellow flags, objective examination, clinical impression, treatment given, home exercises and plan. You can also build your own template.

    Is there a body chart?

Yes. On a physio note you can mark symptoms on a front and back body chart, with a detail for each mark. The chart is saved with the note, fixed once you sign it, and printed in the note PDF.

Does NoteSono tell me if a red flag needs action?

No. NoteSono records the screening you did and what you found. It never assesses risk, suggests a diagnosis or recommends treatment. You stay the clinician.

Can patients see their home exercises?

Yes. Download the programme as a PDF to print or email, or, on Pro, the patient sees their active exercises in the client portal.

    Which plan do I need?

The free plan includes the physio follow-up format and SOAP, with 5 notes a month. Essentials (£14.99 a month) adds the physio assessment format, unlimited notes, home exercise sheets, and pain and activity ratings. Pro (£30 a month) adds the diary, online booking, invoicing, insurer billing and the client portal, where patients see their exercises.

Does it work on a tablet between patients?

Yes. NoteSono works in the browser on phone, tablet and computer, and can be added to your home screen like an app.
